Parent/Guardian Attendance Reporting Form

This form is to be used for all school attendance reporting (Tardies, Absences, Dismissals).  This form is strictly for parent / guardian reporting of attendance.  Students are not permitted to self report.  

As a measure of verification, accountability and safety this document will record the email of the submitting party.

Any files that are uploaded are confidential and only shared with SHS Administration, Health Services and / or the students' School Counselor as needed.

STUDENTS SHOULD CHECK IN WITH THE MAIN OFFICE IF THEY ARE TARDY AS WELL AS BEFORE LEAVING, IF THEY ARE BEING DISMISSED.
